Who may submit?

A named physician or nurse practitioner may submit directly. Authorized clinic staff may submit on behalf of the named physician or nurse practitioner. Use the practice’s referral form above, or an established clinic-approved secure channel.

What counts as a formal referral?

A referral document the clinic has received, or an authorized confirmation that it was received. Completing the practice’s referral form above counts; a queued or sent fax on its own does not.

Can phone advice be requested?

Not through a public route. Where an established clinic channel supports it, phone advice is clinician-to-clinician and does not involve patient contact or booking.

Can an urgent but non-crisis referral be sent?

Not through this website. We have not published urgent or semi-urgent response standards yet, and urgent care needs a faster door than a website.

Can a crisis referral be sent?
